Spay/Neuter Appointment Form

Our free spay/neuter program is for pets only.
This form cannot be used for feral reservations. INSTEAD, please send an email message to PGFeralFix@gmail.com. For $45, feral cats will be altered, given a rabies shot, a first distemper shot, and ear tipped. Feral cats must be brought in a trap (except for young litter, which may come in a carrier). Give us your name, address, and phone number, and tell us how many cats you want to bring in. For each feral cat, please list a name, coloring, and your best guess of sex and age in the email.
